Athletics & Clearances

Please follow the directions to complete your registration and physical to compete at Colony High School.
 

  1. Print the school's physical form. The parent completes one page, and the physician fills out the other when the student gets his or her physical. The physician must be an AD/ DO only. Chiropractic exams are not allowed. Please click here for the required document.
  1. After your child has seen a physician and completed their physical, you will then create an account on the athletic clearance website. If the student-athlete already has an account, he or she must login with their username and password.
  1. Once creating an account or logging back in to the athleticclearance website, complete/ update the student's information (Register - California - Colony High School - Sport of Choice - registration, emergency information, medical history, and district and school procedures) for the new school year.
  1. Upload those two completed and signed physical forms to the athleticclearance website.
  1. Double check all areas on the Athletic Clerance web page.
  1. Submit everything on athleticclearance and wait to get cleared by our Athletic Trainer, Aaron Strout. He will contact you via email.

The Athletic Trainer will remain on campus until 15 minutes after completion of the event.

Monday - Friday
2:30pm - 6pm*
Game days will vary the treatment time availabilities 
All treatments will begin promptly during 7th period, unless otherwise agreed upon with Head Athletic Trainer.
